Karen Hoffman, LCSW

I have been a social worker for over 35 years; I have always practiced in geriatrics. I have worked in many in-patient, and out-patient settings doing different social work programs including chronic kidney disease, cardiac, burn unit; Ombudsman training volunteers for nursing homes, and finally hospice and palliative care for the last 12 years.

I found I really loved working in home hospice care, and from that I found more passion for community based Palliative Care. I was given an opportunity to start a home based palliative care program for a few years out of Phelps Hospice. I really enjoyed helping those with chronic illness and helping to support the patients and their families, as well as having important discussions around goal of care.

I have 3 children, 2 married, and many grand dogs.
I love gardening and traveling, and of course dogs!
